Healthy Babies/Case Management

Healthy babies are more likely to be born to mothers who have early and adequate prenatal care.  There are many mothers who do not receive this care, which automatically places them and their infants in a higher risk category.  In addition there are mothers who receive adequate care but may need these services.

We are pleased to offer a service based on an initial review of the patient's needs and situation. The perinatal social worker identifies and follows these patients and families from admission to discharge.

Referral to a number of services can be offered, including:

WIC (Women, Infants & Children's) certification
Provides health and nutritional services, including supplemental foods, to pregnant women, postpartum women, breastfeeding mothers, infants or children up to 5 years of age who meet eligibility requirements

Healthy Start/Healthy Families Program
Provides prenatal and child case management to Medical Assistance (MA) eligible and low income pregnant women, infants and children under 2 years of age
Community Health Nurse Home Visiting
Hours: Monday through Friday: 8:00 a.m. - 5:00 p.m.

No charge for services, but must have Medical Assistance

Skilled home nursing visits and other resources
This service focuses on achieving a healthy outcome for the mother, family and new baby.  The case manager communicates with both physicians and nurses.  This team approach assures that all needs of our patients and families are met.  For further information about this service call 410-543-7484 or 410-543-7276.
