
Healthy babies are more likely to be born to mothers who have early and adequate prenatal care. There are many mothers who do not receive this care, which automatically places them and their infants in a higher risk category. In addition there are mothers who receive adequate care but may need these services.
We are pleased to offer a service based on an initial review of the patient's needs and situation. The perinatal social worker identifies and follows these patients and families from admission to discharge.
Referral to a number of services can be offered, including:

Skilled home nursing visits and other resources This service focuses on achieving a healthy outcome for the mother, family and new baby. The case manager communicates with both physicians and nurses.
