The Medical Record


The medical record, the property of Peninsula Regional, is kept for the benefit of the patient, the physician and the Medical Center.  The medical record concerns the patient and others.  In keeping with this thought, medical record information is available either electronically, orally or in writing to:

  • Members of the Medical Staff, upon request, and for valid reasons


  • Authorized Medical Center personnel who have valid reasons, and with the permission of the Director of Health Information Management or a delegated representative


  • Physicians without staff privileges:

    • When a patient is under the care of that physician


    • For research with the permission of the Protocol Assessment Committee


    • Other reasons with written patient authorization


    • Waiver from the Investigational Review Board


  • Other Providers, Hospitals and Nursing Homes directly involved in the patient's care


  • In an emergency situation, release may be made without patient consent at the discretion of the Director of Health Information Management or a delegated representative, or written order by staff physician


  • All Others (attorneys, insurance companies, governmental agencies) with proper written patient authorization unless mandated by state or federal regulations
